First Visit

We're here to help! To prepare for your visit, it helps to have the following documents:

• A print out of your completed online registration form
• A list of your current medications
• Your medical and dental insurance cards
• A government-issued picture identifcation card

NTFOS is committed to an exceptional standard of care, the cornerstone of which is careful planning and preparation; therefore, your first visit is usually for a consultation only. This important first visit is the foundation of a smooth and successful surgery. The surgeon will perform an examination, review your health history, explain treatment recommendations, the procedure(s) and after care instructions, and attend to any other items necessary for optimal care. During this visit, we will also help you schedule your surgical appointment.

IMPORTANT: Patients under the age of 18 must be accompanied by a parent or guardian
